A case of dyspnea caused by the right ventricle

A 59-year-old male patient with a history of heart murmur since the age of 5 years without
medical follow-up came to the emergency room complaining of dyspnea at rest and palpitations. From 6 months before admission, he presented dyspnea NYHA (functional class) I, which progressively increased. Transthoracic and transesophageal echocardiography revealed severe subpulmonary obstruction by abnormal muscle bundles forming a fibrous ring, in other words, the diagnosis of a double-chambered right ventricle (DCRV) was made and confirmed by cardiac magnetic resonance imaging. During hospitalization, surgical treatment was decided. Now, the patient has had annual follow-ups for six years, with a favorable evolution.
